3. Increase independence and self-sufficiency
4. Reduce hospitalization and institutionalization
Services:
1. Medical Care (doctor visits, nursing care, medication management)
2. Personal Care (bathing, dressing, feeding)
3. Household Support (light housekeeping, laundry, meal preparation)
4. Companionship and Socialization (conversation, emotional support)
5. Safety and Security (fall prevention, emergency response systems)
Methodology:
1. Needs assessment and client intake
2. Care planning and coordination
3. Service delivery by trained caregivers
4. Regular monitoring and evaluation
Target Beneficiaries:
1. Senior citizens (60+ years) in Pakistan
